Medically Complex Foster Care

We specialize in providing therapeutic foster care to the state’s most medically vulnerable children through our Medically Complex Foster Care program. The children we serve have a wide range of complex conditions.

With the belief that every child deserves a home, we work to provide them with personalized care in a family home.

A Community of Support

It takes a village to raise a child, and we create the community of support children with complex medical conditions need. Our expert team of registered nurses, clinical supervisors, and case coordinators work with the child, their family, and their foster parents, who we call Mentors. Together, we develop an Individual Service Plan which includes:

• 24-hour home-based monitoring
• Therapeutic intervention
• Medication management oversight
• Access to routine family and community-based activities
• Life skills development

Caring Mentors, Loving Home

We carefully screen our Mentors and ensure they have the skills necessary to support the child in their care, providing them with assistance on home modifications and skill development on medical equipment and medication administration. We are with our Mentors every step of the way, helping them provide the children we serve with the care they need. Our nurses are available to answer questions and regularly make house calls, and our clinical coordinators are on-call 24/7 to offer guidance and support.
