Host Home Program
In our Host Home program, adults with intellectual and developmental disabilities live in the community of their choice in the private family home of their dedicated Host Home providers who we call Mentors. We call them Mentors because they are more than caregivers–they are teachers, advocates, and friends.
Personalized Supports
People in our Host Home program are able to participate to the best of their abilities in everyday activities of home and family life. They can learn new skills, discover new hobbies, and explore their community–all while receiving the supports they need. With round-the-clock guidance from our expert team, Mentors provide the individuals we serve with:
- 24-hour home-based monitoring
- Medication management oversight
- Community integration/structured activities
- Life skills development
- Activities of daily living management
- Transportation
- 24/7 on-call support from our team of experts

It’s All about Relationships
We carefully select our Mentors and ensure they have the skills needed to provide exemplary support to the people we serve.
Once a Mentor is ready to welcome an individual into their home, we seek the best match possible by considering the needs and preferences of both the Mentor and the individual, helping to ensure the placement evolves into a long and enriching relationship.
